The Buffalo Therapeutic Riding Center Student Application
Please fill out the information completely. Please remember that the medical forms MUST be filled out by the child's doctor.
The referral form must be filled out by the child’s physician or therapist. Be advised that for safety reasons we can only accept students between the ages of 5 and 18 and cannot exceed 150 pounds.

Please be advised that returning this application does not guarantee your acceptance into the program. The Center’s Board of Directors reviews all applications and has the right to deny acceptance to anyone whose disability may require services beyond the expertise of the Center’s instructors.
Once your application is received, it will be evaluated by our staff. If your application meets our requirements, your child will be invited for a trial unmounted lesson to assess their compatibility with our program.
